All shareholders are required to complete this form. It allows us to confirm the information held in the company’s formal shareholder records, prepare your Shareholders’ Agreement correctly and understand who will be attending the first shareholders’ meeting.

Yes. You must complete the form even if you are unable to attend the shareholders’ meeting. The form is not only an RSVP; it is also being used to confirm the formal details connected with your share.

For an individual share, please enter the shareholder’s full name exactly as it should appear in the company records. A share should only be recorded jointly where it is held by a legally married couple. In this case, please enter the joint name in the format “Mr and Mrs Smith”, rather than listing two separate names. The jointly held share(s) will still carry one vote per share allocation.

For a group share, please enter only the name of the person formally responsible for representing the share and exercising its vote. We do not need the names or details of the other group members.

More than one person connected with a jointly held or group share may attend the meeting. However, voting rights are attached to the share itself, so each share carries one vote. The joint holders or group members should agree in advance who will exercise that vote.

Each share allocation carries one vote, regardless of the number of individual shares included within that allocation. Whether your allocation contains one share or eight shares, you will have one vote at the shareholders’ meeting.
